A penalty surcharge will be assessed for:
(1)
major work performed without a permit, outside the scope of a permit, or after the suspension of a permit;
(2)
work on a property that requires a Notice to Proceed from CHAP performed without a permit, outside the scope of a permit, or after the suspension of a permit; and
(3)
work that continues after the Department issues a Stop Work Order and:
(a)
a permit has not been obtained for the work; or
(b)
the work has not been removed.
