## § 41-5 Form, etc., of citation.

Balt., Md., Code art. 1, § 41-5 (2026)

(a) The civil citation shall be in the form specified by the Chief Judge of the District Court.

(b) The citation shall contain:

  (1) the name and address, if known, of the person charged;

  (2) the violation with which the person is charged, including a narrative statement of the cause for issuing the citation;

  (3) the time when and place where the violation occurred;

  (4) the amount of the fine;

  (5) the manner and time within which the person must either:

    (i) pay the fine specified in the citation; or

    (ii) request a trial on the violation.

(c) The citation also shall contain a certification, signed by the issuing enforcement officer under penalties of perjury, that the facts contained in it are true to the best of the officer’s information, knowledge, and belief.
