## § 15-12 Applications.

Balt., Md., Code art. 15, § 15-12 (2026)

(a) An application for a license must be made in the form the Department requires.

(b) The application must be accompanied by a one-time, non-refundable application fee of $25, to cover the cost of investigating and processing the application.

(c) The application must contain:

  (1) the applicant’s name and address;

  (2) the applicant’s age;

  (3) the type of entertainment for which the license is sought;

  (4) the location or area for which the license is sought; and

  (5) any other information the Department requires.

(d) The application must be verified before a notary public or other officer authorized to administer oaths.
