The statement shall include a schedule of any financial or contractual relationship with:the University of Maryland Medical System; a governmental entity of the state or a local government in the state; or a quasi-governmental entity of the state or a local government in the state; or For each financial or contractual relationship reported, the schedule shall include:a description of the relationship; the subject matter of the relationship; and the consideration.
