All reports must be made in writing, in the form that the Commissioner requires. In addition to any other information specified in this title or required by the Commissioner, all reports must contain the following patient information, to the extent known:name; place of dwelling; date of birth or, if not known, approximate age; sex; race/ethnicity; and occupation.
