An application for a permit must be made in the form that the Commissioner requires. In addition to any other information that the Commissioner requires, the application must contain:the name of the applicant; the location of the property for which the permit is requested; the dates and hours for which the exemption is requested; the equipment that will be used to amplify sound; and a statement as to whether the sound will contain commercial speech.
