Application for a motor vehicle repair facility license shall be made upon a form prescribed by the Director. In addition to such other information as the Director may reasonably deem as necessary for purposes of the implementation, administration, and enforcement of this Subtitle, the application shall state the following: The name of the applicant and, if the applicant is a partnership, the name and address of each partner; or, if the applicant is a corporation, the names and addresses of the principal officers of the corporation and the state in which incorporated, or the name and address of the authorized designated representative for the State of Maryland and the State in which incorporated; The addresses and telephone numbers of all places in the County where the business of the applicant is to be conducted; The number of motor vehicle mechanics, as defined in this Subtitle, employed by the applicant on a full-time, part-time, or intermittent basis; An identification of the particular areas of motor vehicle repair work in which the applicant will engage in business; An identification of the particular areas of motor vehicle repair work in which each of the applicant's motor vehicle mechanics possess technical competence and the number of motor vehicle mechanics employed in each such area; and, The number of the applicant's motor vehicle mechanics who possess valid automotive repair certificates from an automotive repair testing service, the name(s) of the automotive repair testing service(s), and the particular area(s) of automotive repair in which each such motor vehicle mechanic has been certified. Every application shall be signed and certified that the information given thereon is true and correct. The signature shall be that of the applicant if the applicant is an individual; or if the applicant is a partnership or corporation, the signature shall be that of an officer or partner thereof. Every application shall be accompanied by the fee required in this Subtitle, as prescribed in the Table of Fees, section 2-253.63, Attachment A and as published on the DPIE website.
