To change the name of a licensee or a firm on a license certificate and pocket card, a licensee shall submit to the Commission: an application on the form that the Commission provides; the license certificate and pocket card of the licensee; any documentation about the name change that the Commission requires; and a fee set by the Commission. On receipt of the application, fee, and any required documentation, the Commission shall issue, to the licensee, a new license certificate and pocket card containing the new name of the licensee or firm.
