Title 17. Laboratories

Subtitle 2. Medical Laboratories

(a)

An applicant for a license shall submit an application to the Secretary on the form that the Secretary requires.

(b)

An application for a license to operate a medical laboratory shall include:

(1)

The name of the owner;

(2)

The classes of services, complexity of testing, or the tests or examinations that the medical laboratory would provide; and

(3)

Any other information that the Secretary requires.
