An applicant for a license shall submit an application to the Secretary on the form that the Secretary requires. An application for a license to operate a medical laboratory shall include: The name of the owner; The classes of services, complexity of testing, or the tests or examinations that the medical laboratory would provide; and Any other information that the Secretary requires.
