## Title 18. Disease Prevention

### Subtitle 2. Reports; Preventive Actions

#### Section 18-212

(a) The health officer shall keep a record of each report that is submitted to the health officer under Sec. 18-201 of this subtitle.

(b) The record shall:

  (1) Be on the form that the Secretary provides;

  (2) State the date of the report;

  (3) State the name of the physician who submits the report;

  (4) State the disease identified in the report;

  (5) State the name and residence address of the patient named in the report; and

  (6) State the actions taken to prevent the spread of the disease.
