The health officer shall keep a record of each report that is submitted to the health officer under § 18-201 of this subtitle. The record shall: Be on the form that the Secretary provides; State the date of the report; State the name of the physician who submits the report; State the disease identified in the report; State the name and residence address of the patient named in the report; and State the actions taken to prevent the spread of the disease.
