There is a Community Integrated Medical Home Program. The mission of the Community Integrated Medical Home Program is to: Keep Maryland families healthy through the use of innovative mapping tools that allow better targeting of resources to those in need; Coordinate comprehensive services provided by a participating patient centered medical home with public health resources in local communities across the State; and Provide complementary support for qualified individuals between office visits. The Community Integrated Medical Home Program shall be administered jointly by the Commission and the Department.
