A hospital’s discharge process may incorporate established evidence–based practices, including those described in:
(1)
Standards for accreditation adopted by The Joint Commission or another nationally recognized hospital accreditation organization; and
(2)
The Conditions of Participation for hospitals adopted by the Centers for Medicare and Medicaid Services.
