An applicant for registration shall: Submit an application to the Board on the form that the Board requires; Provide documentation of: Enrollment in a doctor of pharmacy program; Graduation from an approved college of pharmacy; or For an applicant who satisfies the requirements under § 12–6D–03(a)(4) of this subtitle, proof of: Educational equivalency; and Oral English competency; Submit to a request for a State criminal history records check; and Pay the application fees set by the Board. The application shall be signed by the applicant.
