(a)
An applicant for a limited lines license as a self–service storage producer shall file a written application with the Commissioner in the form the Commissioner requires.
(b)
The application shall include:
(1)
the physical address of the company headquarters of the self–service storage producer; and
(2)
a list of all self–service storage facilities where the self–service storage producer will conduct business under the limited lines license.
