A health care provider may freely copy, store electronically and subsequently print, or otherwise reproduce the form. A health care provider may: Include on the form additional identifying information about a patient; or Alter the spacing or position of identifying information about a patient. A health care provider may not alter the contents or format of the form other than as provided in §B of this regulation.
