A.
To apply for a loan, an applicant shall;
(1)
Submit a completed application in such form as the Department may require;
(2)
Supply a copy of the latest State income tax return, if appropriate, for each member of the household, or other available verification of any existing household income; and
(3)
Submit any other information or documentation reasonably requested by the Program in order to establish eligibility and qualifications for a loan.
B.
Application Fee. The Program may require an application fee for processing the loan application.
