A.
An eligible employee shall:
(1)
Comply with the Program requirements and terms for receiving an LNYW grant; and
(2)
Respond to periodic surveys for Program evaluation purposes including a survey to be completed at settlement.
B.
Only one person per household may receive an LNYW grant.
