A.
An application for a food desert designation shall be submitted by a local government or local governments.
B.
An application for a food desert designation shall be submitted to the Department on forms prescribed by the Department and in accordance with the schedule that the Department establishes.
C.
An application for a designation shall provide:
(1)
A detailed map and description of the proposed food desert demonstrating the proposed areas within a priority funding area;
(2)
Information and data on the:
(a)
Availability of fresh fruit, vegetables, and other healthy foods in the area;
(b)
Income levels of local residents; and
(c)
Transportation needs of local residents and the availability of public transportation;
(3)
Any comments the applicant has received from the local governments or community; and
(4)
Any other information that the Department considers relevant.
