An application for a food desert designation shall be submitted by a local government or local governments. An application for a food desert designation shall be submitted to the Department on forms prescribed by the Department and in accordance with the schedule that the Department establishes. An application for a designation shall provide: A detailed map and description of the proposed food desert demonstrating the proposed areas within a priority funding area; Information and data on the: Availability of fresh fruit, vegetables, and other healthy foods in the area; Income levels of local residents; and Transportation needs of local residents and the availability of public transportation; Any comments the applicant has received from the local governments or community; and Any other information that the Department considers relevant.
