Regulation .04 Program Requirements.

Youth transitional services shall be made available to all eligible youth and shall include the following components:

(1)

Assessment services;

(2)

Service agreement;

(3)

Case plan;

(4)

Transition plan;

(5)

Case management services; and

(6)

Semi-independent living services.

The local department shall promote youth participation in youth transitional services in order to encourage youth to accept responsibility for:

(1)

Preparing for adulthood; and

(2)

Making the transition from adolescence to successful adulthood.
