Youth who participate in youth transitional services shall have a case plan as described in COMAR 07.02.11. In addition to the components required by COMAR 07.02.11, the case plan shall include: The youth's strength and needs assessment; A description of the youth's current living arrangements; Educational placement; Health care status; Documentation of special needs; A personal budget; Benchmarks; A signed service agreement; and A discussion of progress made by the youth towards the goal or goals identified in the youth transitional services agreement.
