A local department administering youth transitional services shall track:
The number of youth receiving transitional and aftercare services;
The characteristics of the youth receiving transitional and aftercare services;
The type and quantity of services being provided to youth; and
The outcomes for participating youth.
Outcome measures used to assess the operation and effectiveness of the youth transitional services shall include:
Educational attainment;
Employment;
Avoidance of dependency;
Homelessness;
Non-marital child birth;
Incarceration; and
High risk behaviors.
National Youth in Transition Database.
The purpose is to:
Develop a data collection system to track the youth transitional services provided to youth; and
Develop outcomes that measure success in preparing youth for their transition from out-of-home placement to independence.
The Administration shall collect data on youth in two intervals.
The baseline data shall be collected on all youth in out-of-home placement within 20 days of the youth’s 17th birthday to track the needs of the youth.
Follow-up data shall be collected when the youth attains 19 years old and 21 years old.
Data shall be collected in the following areas:
Strength and needs assessment;
Academic support;
Post-secondary educational support;
Career preparation;
Employment programs or vocational training;
Housing education and home management training;
Budget and finance management;
Health education and risk prevention;
Family support and healthy marriage education;
Mentoring;
Supervised independent living;
Room and board financial assistance; and
Education financial assistance.
Outcomes shall measure:
Financial self-sufficiency;
Experience with homelessness;
Educational attainment;
Positive adult connections;
High-risk behavior; and
Access to health insurance.
