Care Record. If an application is accepted, the provider shall develop and maintain a care record for each recipient of the service. The care record includes the: Completed application; Service plan; Records of each redetermination and reconsideration; Records of termination and disposition of the case; and Records of service delivery. Level of Care. The levels of care are: Level I care; and Level II care. Method of Delivery. Respite care services may be delivered either in the residence of the individual with the developmental or functional disability or in a respite care facility.
