Role of the provider case worker;

Specific tasks to be carried out by treatment parents during placement;

Long-term goals of treatment, including criteria for discharge, projected length of stay in the program, projected post-treatment, and aftercare services; and

Identification of treatment team members who will assist in the provision of planned care.
