The application documents shall include: The name of the county and, if applicable, the municipality in which the racetrack is to be located; The legal description of the racetrack site; The name and address of each person who has held title to the real property during the 5-year period preceding the date on which the application documents are filed; The name and address of each person who holds a mortgage or other security interest in the real property; An unconditional commitment by the owner of the real property for a title insurance policy; An aerial photograph of the racetrack site; A photograph of the site from each roadway adjacent to the site; and A description of the current commercial and industrial uses for all property within a 1-mile area around the racetrack site.
