Staff of the assisted living program shall complete an incident report within 24 hours of having knowledge that an incident, as defined in Regulation .02B(39) of this chapter, occurred. The assisted living program shall make incident reports available on the premises to the Department and any government agency designated by the Department. All incident reports shall include: Time, date, and place; Individuals present; Complete description of the incident; Response of the staff at the time; and Notification to the: Resident or, if appropriate, the resident representative; Resident’s health care practitioner, if appropriate; Delegating nurse; Licensing or law enforcement authorities, when appropriate; and Follow-up activities, including investigation of the occurrence and steps to prevent its reoccurrence. The assisted living program shall notify the OHCQ within 24 hours of a resident death resulting from: Abuse; Neglect; Wandering; Elopement; A medication error; Burns; or Any injury incurred at the assisted living program.
