The following are not covered:
A.
Services by an out-of-State nursing facility unless the Department and the nursing facility execute a provider agreement;
B.
Audiology services;
C.
Services reimbursed under Title XVIII of the Social Security Act; and
D.
Services for which payment is made directly to a provider other than the nursing facility.
