Definition. “Transition service” means a service that is: Not otherwise available under the Program; Approved in the plan of service; and Rendered to assist the participant in transitioning from an institution or a provider-owned residence to a home or community-based residence. Transition services may include all or some of the following: Security deposits; Essential furnishings and moving expenses; Set-up fees or deposits for utility services; and Other health and safety assurances. Transition services may not include recreational items including, but not limited to: Televisions; Cable television access; or Gaming systems.
