An applicant for a cord blood transplant center grant shall:
A.
Submit a completed application to the Department by the due date listed in the annual request for proposals; and
B.
Include in the application packet the following information as specified in the annual request for proposals:
(1)
Executive summary;
(2)
Statement of the need for funding;
(3)
Proposed work plan for the grant period;
(4)
Brief professional resume or curriculum vitae of key staff involved in the grant activities;
(5)
Description of how the requested funding will further the institution’s goal of being recognized as a cord blood transplantation regional center of excellence;
(6)
Timeline detailing the expected start and completion dates for each activity to be conducted under the grant;
(7)
Organizational chart indicating where the work under the grant will be located within the institution;
(8)
Statement that the grant money will not be used to supplant any existing funding;
(9)
Line item budget for the grant; and
(10)
Written budget justification.
