Regulation .06 Proof of Education.

Each optometrist is responsible for obtaining proof of hour credits and maintaining this documentation for inspection by the Board for 4 years after the date of renewal.

To obtain credit, one's participation shall be attested to in writing by someone in charge of the program.

Failure to do so shall be grounds for nonrenewal of the license.
