An applicant shall complete and file an application on the form provided by the Board on request. The applicant shall submit the following information: Full name, residence, and date and place of birth; A recent photograph of the applicant; Letters of recommendation from three individuals, excluding immediate family members, who can attest to the applicant's character and reputation; All final professional transcripts of colleges or university schools of optometry; The application fee specified in COMAR 10.28.07; Satisfactory evidence of having completed a criminal history records check in accordance with Health Occupations Article, §11-302.1, Annotated Code of Maryland; and Preprofessional transcripts. Application fees are not refundable. Applicants shall be notified whether they meet the standards adopted by the Board and qualify for licensure.
