The following fees are applicable to athletic trainers. Initial licensure fee — $200; License renewal fees: License renewal fee — $135; and Maryland Health Care Commission (MHCC) fee — as determined by MHCC under COMAR 10.25.03; Reinstatement fee — $200; Evaluation and treatment protocol — $100; Written verification fee — $25; and Replacement of license fee — $25.
