The following fees are established by the Board: Pharmacist Fees. Pharmacist examination fee — $150; Pharmacist reciprocity fee — $300; Pharmacist renewal fee — $225; Pharmacist reinstatement fee for up to 2 years after license expiration, payable in addition to renewal fee — $300; and Pharmacist reinstatement fee for more than 2 years after license expiration, payable in addition to renewal fee — $315. Pharmacy Fees. Pharmacy initial fee — $700; Pharmacy renewal fee — $500; and Pharmacy — late fee (payable if renewal fee is received between May 2 and June 30) — $200; and Pharmacy reinstatement fee (payable if renewal fee is received after June 30) — $550. Distributor Fees. Distributor initial fee — 1,750; Distributor renewal fee — $1,750; and Distributor reinstatement fee after May 31 — $1,500. Pharmacy Technician Fees. Pharmacy technician registration fee — $45; Pharmacy technician renewal fee — $45; Pharmacy technician reinstatement fee — $45; Review of pharmacy technician training programs submitted to the Board for approval — $100; and Review of pharmacy technician training programs submitted to the Board for approval that include a didactic portion that is comprised entirely of a didactic program that has already been approved by the Board — no charge. Pharmacy Intern Fees. Pharmacy intern registration fee — $45; and Pharmacy intern renewal fee — $45. Miscellaneous Fees. Duplicate registration fee — $10; Duplicate pharmacist license fee — $30; Duplicate pharmacy permit fee — $10; Duplicate wholesale distributor permit fee — $30; Written verification of good standing fee — $25; Returned check fee — $35; Roster printed on labels fee — $150; and Failure to maintain current address fee — $25.
