If not addressed in the initial order, the infusion pharmacy shall verify an end of therapy order. The infusion pharmacist shall: Communicate with the licensed authorized prescriber of record or agent of the prescriber of record to verify: An end of therapy order, if not addressed in the initial order; and Parenteral infusion access device disposition, including orders for removal or line maintenance; and Forward the most current medication list to: The licensed authorized prescriber of record; and To the patient or caregiver. If appropriate, the infusion pharmacy shall arrange with the patient or caregiver for pick-up of medical equipment.
