The Board shall reinstate the license of an individual who has failed to renew a license which expired for any reason if the individual: Otherwise is entitled to be licensed; Complies with the continuing education requirements established by the Board for this purpose; Submits the application for reinstatement; Submits the continuing education report form with copies of the required continued education documentation as set forth in COMAR 10.42.06; Pays to the Board a reinstatement processing fee in COMAR 10.42.05; and Applies to the Board within 5 years of the date the license expired. Failure to Reinstate Within 5 Years. The Board may not reinstate the license of a social worker who fails to apply for reinstatement within 5 years after the license expired. The Board may issue a license after 5 years if the individual: Is otherwise entitled to be licensed; Submits the required application; Pays all applicable fees set forth in COMAR 10.42.05; Submits an official transcript indicating the date the degree was awarded or conferred; Submits three general professional references; and Passes the respective examination required for initial licensure.
