In addition to the requirements outlined in Regulation .09 of this chapter, for a dentist to obtain a Class I, Class II, or Class III permit, each facility in which the dentist wishes to administer an anesthesia technique shall pass a facility evaluation in order to attain: A level beyond anxiolysis; Moderate enteral sedation; Moderate parenteral sedation; or Deep sedation and general anesthesia.
