#### Regulation .01 General.

*COMAR 10.44.20.01*

A. A person shall pay a fee as set forth in Regulation .02 of this chapter by check or money order.

B. The Board shall provide a copy of this chapter:

  (1) Along with any application form or renewal form; and

  (2) To any other person on request.

C. The Board shall refund the initial licensure or renewal fee, less a $50 processing fee, to a dentist or a dental hygienist who does not qualify for initial licensure or renewal.

#### Regulation .02 Fee Schedule.

The following fees are established by the Board:

A. Initial application fee:

  (1) Dentist . . . $450;

  (2) Dental hygienist . . . $275;

B. Limited license fee:

  (1) Dentist . . . $225;

  (2) Credentials processing (foreign dental school graduate surcharge) . . . $75;

C. Teacher's license fee:

  (1) Dentist . . . $225;

  (2) Dental hygienist . . . $225;

  (3) Credentials processing (foreign school graduate surcharge) . . . $75;

D. Temporary license application fee — dental hygienist . . . $75;

E. Application fee for dentists licensed in another state . . . $450;

F. Application fee for dental hygienists licensed in another state . . . $275;

G. License renewal fee:

  (1) Dentist . . . $560;

  (2) Dental hygienist . . . $182;

  (3) Teacher's license . . . $225;

  (4) Dentist late renewal fee (July 1 through July 31 of the year in which the renewal is due) . . . $300;

  (5) Dental hygienist late renewal fee (July 1 through July 31 of the year in which the renewal is due) . . . $150;

H. Dental radiation technologist certification fee:

  (1) Certification renewal . . . $68;

  (2) For the renewal period ending March 1, 2017, a one-time 1-year certificate for dental radiation technologists whose certificates end in an even number . . . $34;

  (3) Certification reinstatement . . . $118;

  (4) For the period March 2, 2017 through April 1, 2017, for the 2017 renewal period, a late renewal fee for dental radiation technologists whose certificates end in an even number . . . $25;

  (5) On and after April 2, 2017, for the 2017 renewal period, a certification reinstatement fee for dental radiation technologists whose certificates end in an even number . . . $59;

  (6) For the period March 2, 2018 through April 1, 2018, for the 2018 renewal period, a late renewal fee for dental radiation technologists whose certificates end in an even number . . . $25;

  (7) On and after April 2, 2018, for the 2018 renewal period, a certification reinstatement fee for dental radiation technologists whose certificates end in an even number . . . $59;

  (9) Initial application fee for applicants holding an active license or certification to practice dental radiation technology in another state . . . $20;

S. Certificate fee for a treating dentist to treat a patient at a practice location other than their own, at which anesthesia and sedation is administered to the patient:

  (4) Renewal of additional location certificate . . . $300;

T. Single fee for dental hygienist recognition as qualified to administer local anesthesia, or qualified to administer nitrous oxide, or qualified to prescribe and administer medication, or any combination thereof . . . $50;

U. Certificate fee for a treating dentist to allow a certified registered nurse anesthetist to administer anesthesia and sedation at a specific practice location:

  (1) Initial application . . . $1000;

  (2) Renewal . . . $300;

  (3) Additional location certificate . . . $300;

  (4) Renewal of additional location certificate . . . $300;

V. Inactive status application fee (per renewal):

  (1) Dentist . . . $150;

  (2) Dental hygienist . . . $75;

W. Board fee for full permit to dispense prescription drugs . . . $75;

X. Required Division of Drug Control inspection fee for full permit to dispense prescription drugs . . . $1,000;

Y. Limited permit for dispensing prescription drugs:

  (1) Initial application . . . $75;

Z. Application reprocessing fee . . . $50;

AA. Corporate name application fee . . . $150;

BB. Returned check fee . . . $25;

CC. Late fee for failure to notify the Board within 60 days of change of address . . . $10;
