Legible certificates of program completion shall be provided to those who complete the program and shall contain at least: The full name of the sponsor; The full name of the program; The dates of program presentation; The location of the program, including whether the program was presented electronically; The number of continuing education hours awarded, which may not exceed those approved by the Board; The printed name and signature of the presenter or speaker; A statement that the program is approved by the Board; The Board’s program approval number; and A statement that the sponsor maintains responsibility for the program.
