A dentist or a dental hygienist who has been granted a site-specific volunteer dentist or dental hygienist's license shall immediately surrender the license to the Board if the dentist or dental hygienist ceases to: Provide volunteer dental or dental hygiene services at the institution named on the license; or Be covered by malpractice insurance from or through the institution named on the license. A dentist or a dental hygienist who has been granted a volunteer dentist's or dental hygienist's license that is not site-specific shall immediately surrender the license to the Board if the dentist or dental hygienist ceases to be covered by malpractice insurance.
