A.
The purpose of the Commission is, through community health resources, to:
(1)
Increase access to health care services;
(2)
Redirect nonemergency emergency room use to community health resources;
(3)
Develop coordinated, integrated systems of community-based care; and
(4)
Assist patients in establishing a medical home with a community health resource.
B.
A key objective of the Commission is to reduce health disparities in the State.
