The purpose of the Commission is, through community health resources, to: Increase access to health care services; Redirect nonemergency emergency room use to community health resources; Develop coordinated, integrated systems of community-based care; and Assist patients in establishing a medical home with a community health resource. A key objective of the Commission is to reduce health disparities in the State.
