A licensee shall retain all complaint records and documentation of the investigations and corrective actions taken by the laboratory. A licensee shall ensure that complaint documents include at least the: Name and organization of complainant, if applicable; Date of complaint; Details of complaint; Corrective action and resolution; and Notification of the complainant as to the resolution of the complaint. A licensee shall forward all complaints to the laboratory director.
