The State agency shall provide an application package to a vendor that requests it. The application package shall consist of the following: An overview of the WIC Program; The minimum requirements for authorization; The WIC Authorized Foods List; The Minimum Required Stock List; The Application Checklist; The eWIC Readiness Assessment; The Vendor Application Price List; The Authorized Infant Formula and Medical Foods Supplier Directory; Instructions for completing the application for vendor authorization; An application for vendor authorization; and Two vendor agreements.
