## Subtitle 56 — Board of Dietetic Practice

### Chapter 01 — Licensure

#### Regulation .12 Change of Mailing Address, Legal Name, or Email Address.

*COMAR 10.56.01.12*

A. A licensed dietitian-nutritionist shall give the Board written notice within 30 days of a change of:

  (1) Mailing address;

  (2) Legal name; or

  (3) Email address if the licensee has previously submitted an email address to the Board.

B. Failure to notify the Board of any change of mailing address, legal name, or email address within 30 days of the change may result in a $50 fee as set forth under COMAR 10.56.02.
