The following fees are established by the Board: Residential Child Care Program Administrators: The Board shall prorate the initial application fee for applicants who apply for initial certification in the following manner: Within 2 years to 18 months of the renewal date . . . $200; Within 18 months to 1 year of the renewal date . . . $150; Within 1 year to 6 months of the renewal date . . . $100; and Within 6 months of the renewal date . . . $50; State Comprehensive Examination (charged each time an applicant takes the examination) . . . $125; Biennial Certificate Renewal . . . $200; Reinstatement Fee . . . $200; Acting Capacity Approval—$75; and Authorization to serve as the residential child care program administrator for 2 individually licensed organizations—$100; Residential Child and Youth Care Practitioner: Initial Application . . . $50; Biennial Certificate Renewal—$ 50; and Reinstatement Fee . . . $50; Other Fees: Application for approval of a continuing education program of studies . . . $100; Fine for failure to notify the Board within 30 days of a change in the: Name of the certificate holder; Home address of the certificate holder; Electronic mail address of the certificate holder; or Name or address of employer or business connection of the certificate holder . . . $50; Returned check fee . . . $25; Diskettes, Labels or Rosters of Certified Individuals.
