An MCO shall provide medically necessary fertility preservation services for the following individuals: Individuals who have iatrogenic infertility; Individuals who are receiving medical treatment that may directly or indirectly cause iatrogenic infertility; or Individuals who are receiving gender-affirming treatment. Fertility preservation services include sperm and oocyte cryopreservation and evaluations, laboratory assessments, medications, and treatments associated with sperm and oocyte cryopreservation. Fertility preservation services do not include the storage of sperm or oocytes.
