Two or Three Axle Dump Service Vehicles. The original application for registration shall be accompanied by: A dump service certification (Form VR-98); A sideview photograph of the vehicle; and The fee as set forth in Transportation Article, §13-919, Annotated Code of Maryland. Four or More Axle Dump Service Vehicles. The original application for registration shall be accompanied by: A dump service certification (Form VR-98); A sideview photograph of the vehicle, clearly showing the lift axle; The manufacturer's, second-stage manufacturer's, or authorized dealer's certification that the vehicle is in compliance with the requirements of COMAR 11.15.27.05; and The fee as set forth in Transportation Article, §13-919, Annotated Code of Maryland.
