A person in interest requesting to correct or amend a personal record shall: Make the request in writing; Identify the personal record to be corrected or amended; State the precise correction or amendment requested; State the reason for the correction or amendment; and Direct the request to: The custodian of the record; or If the custodian is unknown, the Executive Director of the Human Resources Division for the Department of Public Safety and Correctional Services.
